Doctors and centres who refer patients to you. Staff pick them under “Referred by” on the form, where their details are locked, so correct them here. Deleting one never changes a form already saved.

Paper copies per Form F

Some districts don't require a patient copy. This changes Form F printing and its “Print” labels. Form G always prints 3 copies.

Print workflow

Split print issues the form in two passes: print the declaration for signing before the scan, then print the result sheet after. Leave on Single unless your clinic is piloting the split workflow.

Procedures this clinic is licensed for

Only change this if your clinic holds a licence for invasive procedures under the PC&PNDT Act. Every form still starts as a normal ultrasound; Section C is offered when you enter the scan result, for the patients who need it. Licensed clinics use the split print workflow for every form — the declaration is signed before the scan, which is what leaves room to add Section C afterwards.

Procedures done at this clinic

Leave on Ultrasound only unless this clinic also does other imaging. Radiology adds field 11 to the form, so staff can record another procedure for the patients who have one. Every form still starts as Ultrasound, and forms already saved keep the procedure they record.
